ELIGILIBITY: Elderly, chronically ill, disabled adults TYPE OF CARE AND SERVICES: Socialization, recreational activities, whirlpool bathing, hair and nail care, personal care, exercise, nutrition, health monitoring, nursing treatments and case management.
